Nurse Clinician - Care Management - Population Health - HSSB
Education Requirements
• Baccalaureate degree in Nursing required.
• Current license to practice nursing.
Experience Requirements
• Reasonable (within the last 1-3 years) nursing experience is required.
• Demonstrated expertise in specific clinical area of practice required.
Desired Qualifications
• Experience in care management or care coordination.
• Experience in community health or public health settings.
• Experience in behavioral health.
• Demonstrate job-related experience with fostering a welcoming and respectful work/academic environment and/or otherwise demonstrate their ability to foster a welcoming and respectful work/academic environment for people from all types of backgrounds and viewpoints
• Knowledge of chronic disease management.
• Experience applying motivational interviewing principles to effectively engage and motivate patients.
• Experience in program development.
• Certification in care coordination, care management, or related area.
