NURSE CLINICIAN - INPATIENT CASE MANAGEMENT

 

Positions available include:

  • 100%,  Days, Monday - Friday, 8 hour shifts between the hours of 7:00AM and 7:00PM as well as weekend and holiday rotations.
  • 100%,  Days, Monday - Friday, 8 hour shifts between the hours of 7:00AM and 7:00PM as well as weekend and holiday rotations.

 

Function as a clinical nurse expert within the Medical Surgical Services Units and care coordinator for the assigned patient population facilitating seamless transitions across the continuum of care. The nurse functions as an administrative and clinical liaison for individuals to optimize quality health care delivery using clinical assessment/screening tools, education reinforcement, follow-up on the plan of care recommendations and guiding the utilization of cost-effective resources. This position will manage a specified population and work closely with other care coordinators during transitions of care as well as other interdisciplinary team members across the Continuum of Care.


REFERENCES: Five professional references will be requested and required at a later step in the recruitment process.

     Required Education/Experience includes:
·   A Baccalaureate degree in Nursing is required.
·  Current license to practice nursing in Iowa is required by date of hire.
Desirable Qualifications includes:
·  Two years recent adult medical/surgical inpatient experience, including care coordination is required.
·  Proficiency in computer applications including Microsoft Excel, Word and PowerPoint is required.
·  Excellent written and verbal communication skills are required.
·  Demonstrated excellent organizational skills, critical thinking, and problem solving skills are required.
· Demonstrated expertise with adult patient/family discharge planning process is required.
·  Experience working with an interdisciplinary team is required.
· Previous case management experience is desired.
· Certification in case management is desired.
·  Previous experience with EPIC is desired.
