The University of Iowa Healthcare Care Coordination Division is seeking a Social Worker III to provide direct social work services including assessment, treatment, interventions, and recommendations to facilitate movement through the continuum of care to Adolescent and Young Adult (AYA) oncology patients as well as their families on both inpatient and outpatient settings. The AYA program services cancer patients age 13-39 old and their families.

Within the Care Coordination Division and Holden Comprehensive Cancer Center the Social Worker will plan, develop, implement, and evaluate comprehensive social work services for adolescent and young adult patients with complex and/or specialized needs. This role will also participate in and provide the social work perspective to interdisciplinary activities, as well as provide social work treatment, individually or in groups, to patients and clients and their families with complex emotional or social problems. 

This is a two-year specified term position and may be eligible for extension based on funding.

Responsibilities to include:

  • Provide high quality patient care for Adolescent and Young Adult cancer patients ages 13-39 with complex or specialized needs including evaluation, assessment, and case work appropriate for patient’s condition and age.
  • Perform transition planning activities and psychosocial assessment at onset and throughout the patient’s hospitalization utilizing clinical skills to create and update personalized transitional care plans.
  • Prepare psycho-social histories, participate in clinical staff diagnostic and treatment staffing, interpret patients’ and clients’ and their families’ psycho-social functioning and needs, and recommend and plan social treatment methods to meet those need
  • Plan for and provide consultation to medical or clinical service staff and community social agencies regarding the handling of medical-social or other social functioning problems of individual of groups of patients and clients and their families
  • Participates in the development of goals for the AYA Program.
  • May represent AYA in monthly Teen Cancer America webinars and other national committees. 
  • Attend meetings and participate on committees and support groups as appropriate; serves as a member of the AYA Core team and AYA Steering Committee.
  • Assists patients in completing AYA Enrollment form and documents relevant information.
  • Maintains EPIC documentation and metrics of all AYA enrolled patients.
  • Shares pertinent information with AYA Nurse Coordinator, AYA Patient and Family Life Specialists and other team members on a continual basis regarding AYA enrolled patients
  • Participate in Care Coordination initiatives or other projects according to departmental and organizational monitors.
  • Function as a liaison between the inpatient unit and community-based resources on an as needed basis.
  • Contribute to the formation of the department, hospital or clinic policies affecting patient and client care, training programs and departmental and professional growth.
  • Participate in departmental or clinic staff meetings and planning sessions and contribute to the solution of department of clinic management problems.
  • Participate in the development, strengthening and delivery of needed health and social services.
  • Advocate for the unique needs of AYA patients, families, and staff.
  • Establish new patient programming and support that specifically targets the adolescent and young adult oncology population and is based on evidence-based standards.
  • Write case reports covering on-going social services for inclusion in the patients’ or clients’ clinical records, maintain statistical records of social service activity.
  • Attend and participate in courses, meetings, conferences and workshops in attainment of new or advanced social work practice skills.
  • Maintain effective working relationships with faculty, staff, students and the public.

Percent of Time: 100%
Schedule: Day shifts, 8-hour shifts, between the hours of 8:00 AM and 4:30 PM, Monday - Friday.  Occasional holiday and weekend coverage.

Required Qualifications for SW III

  • Master’s degree with a major concentration in social work

  • 6 months-1 year of clinical social work experience

  • A license to practice social work in Iowa

  • Excellent written and verbal communication skill

Desirable Qualifications for SW III

  • Some familiarity with social work undergraduate education

  • Social work experience in a health care setting with the appropriate patient population and their families.

  • Experience with identifying and navigating financial resources, including insurance coverage options, changes related to the federal and state health programs, and the disability process.

  • Experience working with community resources, home healthcare, nursing facilities and other options available for patients in outpatient settings

  • Experience in a fast paced, high volume work environment

  • Care coordination experience or case management skills in a health care setting as part of an interdisciplinary team including experience utilizing an electronic medical record to assist with management of non-adherence, post-care follow-up and referrals to appropriate resources.
